Changes for version 0.004

removed dependencie on CGI::Application::FastCGI (removed experimental feature)
update documentation to reflect changes in CGI::Application::Structured::Tools v 0.008
- change default config file name and remove need to export ENV var.
- fixed bug in generation of db schema name
- moved db schema name to config file

cpanm CGI::Application::Structured

perl -MCPAN -e shell
install CGI::Application::Structured
