Changes for version 0.12 - 2005-03-01

[INCOMPATIBLE CHANGE] Get rid of the dual valued scalar exception idea. The way it was implemented meant that you didn't get the line number information about where the exception actually happened.
Allude to Win32 compiler problems in README.
Remove some compiler warnings.
Cope with either a glob or a reference to a glob in StartDocFile().
