Changes for version 1.15

Improved default for ->ctcmd() method in void context (now same as specifying 2).
Added ->fork-exec method (syntactic sugar to let you make it clear that you aren't using ->ctcmd or ->ipc modes).
Fixed bug involving ->stdout(0) and ->ctcmd mode.
Added scripts to benchmark fork/exec vs ctcmd modes.
