Changes for version 0.45

Fixed a bug (in 0.43+) where the Cancel button becomes useless when server-side validation is failed in render_as_form (Reported by Stewart Heckenberg). The 'Cancel' button now sets the value of a special hidden field named '[form_id]_submit_cancel' to 1 in order to bypass the generated Javascript form validation function.
