Upload handler implementation for Apache.

• onstart - upload has started, this is the first request that comes thorugh

• onfilename - we have received filename in the disposition header

• onheader - we have a header

• ondata - additional data has come through

• oncancel - the upload was cancelled

• oncomplete - the upload is now complete
